The Cultists are among the most mysterious factions in Fallout 76. Their beliefs revolve around the Mothman, a cryptid they worship as a divine being. Long before the Great War, a cult dedicated to the mysterious creature had already formed in the town of Point Pleasant. After the bombs fell, this belief evolved into several fanatical sects that practice their religion with unwavering devotion—and often with extreme violence.

Today, Cultists can be found throughout Appalachia, establishing shrines and camps in remote locations where they perform rituals and fiercely defend their sacred sites. While not every sect follows the exact same doctrine, all share a deep reverence for the Mothman and the conviction that they serve a higher purpose. Their unique beliefs, rituals, and connection to Appalachian folklore make the Cultists one of the most distinctive hostile factions players encounter in Fallout 76.

A key location in Cultist lore is Lucky Hole Mine. Hidden deep beneath the mine lies a secret shrine dedicated to a mysterious creature commonly referred to by the community as the Interloper. Whether this being is an ancient deity, a mutation, or somehow connected to the Mothman has never been officially explained. However, the altars, sacrifices, and occult symbols found throughout the mine strongly suggest that at least some Cultist sects regard the Interloper as a sacred entity alongside—or perhaps even above—the Mothman. To this day, Lucky Hole Mine remains one of the most mysterious locations in Appalachia and an important piece of Fallout 76's darker lore.
